We rely on volunteer providers to deliver care to the needy in our community who fall between the gaps of our healthcare delivery system. Our Volunteer Provider Network is an assortment of providers who donate their time to see patients in their own office. Care coordinators at CHC connect patients with providers. They provide financial screening and eligibility services, connecting patients with existing state services wherever possible. The Care Coordinators monitor the flow of patients into each office, and let providers set limits to the number of patients that will be seen in their office every month. They also assist in reducing ancillary costs wherever possible, and are a great source of other resource information. If you would like to become a CHC network provider, please fill out the attached files and return to the CHC office.
