Benefits of Providing Donated Medical Services

 

• You determine how many clients you will see.
• Help needy individuals in our community in your own office.
• All individuals are screened for eligibility to be:

•    -residents of Utah County
     -living at 150% of the federal poverty level or below 

• Option of referring your own walk-ins to our program for financial screening and coordination of care upon qualification.
• Interpretation services for clients who need it.
• Care coordinator assigned to each CHC client we send who:

•    -schedules appointments 
     -coordinates efforts with labs/other providers as needed
     -makes appropriate referrals to outside social service agencies
