Service Overview

At Community Health Connect our mission is to improve access to quality health and dental care for low-income uninsured men women and children.  We accomplish this goal through the coordination of a Volunteer Provider Network, providing health education and by maximizing existing community resources.

 

Our care coordinators connect patients with doctors and dentists willing to provide care on a charity basis.   They provide eligibility screening, coordinate appointments between providers and patients and also coordinate ancillary care needs.  Though free care is never guaranteed, the care coordinators work to minimize costs for eligible clients.

 

We work hard to guide patients through the complex health system and try to teach them how to navigate on their own.  In addition we provide educational programs, outreach, information, and referrals to the low-income, uninsured community.  For details on these resources click on the link on the left.
