Dia de Salud

Dia de Salud is a weekly Spanish health forum that provides screenings, education, resources, and social support for chronic disease prevention and control. It is a gathering place for friends and families to relax and enjoy healthy food, learn, share ideas, offer encouragement and take steps to improve their health, both individually and collectively. One-on-one counseling and personal goal setting with a trained health professional is offered to persons having diabetes and hypertension. Presentations delivered by a variety of community health experts range from mental and emotional wellness to environmental health and nutrition. Presentations are supported by activities such as healthy food preparation classes and physical fitness instruction. Weekly meetings take place every Tuesday evening at Centro Hispano, 819 South Freedom Blvd (200 West) Provo, UT from 5:00 to 7:00 pm.  For more information contact Danelle Holdaway, 818-3015 danelle@communityhealthconnect.org

Health Fairs

Each year Community Health Connect participates in multiple health fairs throughout Utah County. These public outreach events are coordinated by various partnering agencies and are hosted in a variety of settings including schools, libraries, and hospitals. At health fairs CHC disseminates information on diabetes, oral health, and community health resources. Cooperating with student volunteers from local universities and trade schools, CHC staff gather up-to-date health information and develop presentations on topics such as nutrition and physical activity. Utah County Diabetes Coalition

The Utah County Diabetes Coalition was organized to heighten diabetes awareness, prevention and control measures as well as emphasize the health benefits of nutrition and physical activities. In order to accomplish it mission, the UCDC has access to community resources that collaborate to serve those affected by diabetes in Utah County. We also organize the annual St. Patrick's Diabetes Fair, and disseminate the Utah Diabetes Control management information literatures throughout Utah County.

 

Walking Strong Native American Community Coalition

In collaboration with organizations of the Native American community, WaSNACC seeks to promote, protect, and preserve the health of Native Americans. This is accomplished by utilizing traditional and contemporary methodologies that promote increased physical activity and informed healthy lifestyle choices. Health awareness is raised by actively disseminating health information at Native American gatherings, by providing health education classes, and by organizing physical activities such as trail walks, mall walking clubs, water aerobics, and native dance sessions.

Utah County Dental Varnishing Project

Community Health Connect (CHC) partners with Intermountain Healthcare, the Utah County Health Department, and volunteers from local colleges and schools to provide an oral health education program in Title I schools throughout Utah County. The project includes an engaging presentation focused on keeping your mouth clean, properly brushed and flossed. It also encourages children to eat healthy foods that are good for their teeth. Following the presentation each child receives a dental kit with a toothbrush, toothpaste, and dental floss. All children having prior parental consent are then given an application of fluoride varnish. Those receiving the varnish are also screened for dental caries and more serious problems. Children requiring further assistance who are without dental insurance are sent to Community Health Connect, where they are linked with a dentist who provides services on a charity basis.

Migrant & Seasonal Farm Worker Outreach

In Collaboration with the Utah Farm Worker Program and the Utah Migrant and Seasonal Farm Worker Coalition (UMSFW) cooperating agencies, Community Health Connect works to improve access to health information and resources for farm workers. This is accomplished by providing health education and screenings at approved work sites throughout Utah County and by developing culturally and linguistically appropriate materials including maps, policies and contact information for the Payson Family Health Clinic and other key service sites.  For more information contact Kirsti Vogler at 885-2222 or kirsti@communityhealthconnect.org.
